NY State Insurance Fund (NYSIF) uses a specific type of certificate form known as the U-26.3 form. It is the State’s equivalent to the C-105.2. For that reason, all certificates have to be issued via the NYSIF online portal.
NYSIF offers three ways for clients to obtain a certificate of insurance:

The quickest and most convenient way to obtain a certificate is to log on to their NYSIF online account and create one.
- Email certificates@nysif.com. When sending an email, please ensure to include the following information:
- Your name and policy number in the subject line
- Certificate holder name and address in the body of the email
- Call NYSIF with your policy information at 888-875-5790 to have NYSIF email you a certificate within 48 hours.
